A 60yr old female presented to the OPD with chief complaints of pedal edema since 10 days, decreased urine output since 10 days and fever since 10 days.

              POSITIVES:

  • personal history was well presented in a chronological period order.
  • both general and systemic examination details were given properly.
  • investigations were clearly mentioned, with pictures of the reports and the dates on which the investigation were done which was easy to understand the patient 
  • the treatment plan was well described with the dates on which drugs were administered
  • presentation was very well presented and made easy to understand and analyse 
              NEGATIVES:
  • the patient's history should have been presented in a better way by including bullet points 
  • Reason should have been given for drugs.
  • The case feels incomplete as there is no updates after 23 June and no discharge summary.
  • Question 5 - Please reflect on and share your telemedical learning experiences from the hospital as well as the community patients over the last month especially when you were E-logging  
  • It is very nice to have an opportunity like this.The clinical assignment given made me aware about the case history taking and the way to present a given a case.
  • I have also learnt how to analyse the given data about the patient have also learnt how to analyse the given data about the patient
  • I am really glad that our college had come up with eblog idea, which is encouraging and inspiring us to learn more and more.
  • This is a very great experience.
